Planning your first visit to Frederick Gastroenterology Associates

On this page, you'll find the information you need to get started, such as a map and directions to our Frederick office. To save you time, we've also included our new patient forms, which you can print out and complete prior to visiting our office.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this website. Also, take some time to review our staff page and familiarize yourself with the doctors. We look forward to your first visit.

Prior to your appointment we ask that you log on to our patient portal to enter your medical history. If you have scheduled an appointment and have not received a portal invitation please contact the office. If you do not have internet access please complete the patient registration forms and and bring them with you for your appointment. The authorization for verbal disclosure form will allow us to communicate medical information to persons designated by you, i.e., your spouse or relative.

Please bring your insurance cards, a credit card, a photo identification card, prescription card, a valid referral if one is required by your insurance plan, and your co-payment to your appointment.

Bring copies of any previous records or studies (laboratory tests, radiology tests, consultation reports, procedure reports, pathology reports) that have been performed or have your Primary Care Physician send them directly to us.

If you have previously been under the care of another gastroenterologist, please request a copy of your records. You will find a medical records release form on this website.

If you are unable to keep your scheduled appointment, please call our office at (301) 695-6800. We require notification from you to cancel or reschedule your appointment. In the event you do not notify us, there will be a rescheduling fee of $50.00.

Appointments

Patients are seen by appointment only. Appointments can be scheduled by calling during regular office hours. If you find it necessary to change an appointment, please contact the office at least 24 hrs. in advance. This courtesy enables us to schedule other patients needing treatment. Occasionally, we will run behind schedule due to unforeseen emergencies. If this happens, we ask your understanding and patience. We know you have many choices when choosing a Gastroenterologist in Frederick, MD. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Prescription Refills

Please contact your pharmacy for prescription refills. Your pharmacy will send their request to our office via e-prescribe or fax. Many medications require pre-authorization from your insurance company which can take up to 72 hours. Please make sure to give your pharmacy enough time to get your prescription refilled. We will automatically contact your pharmacy regularly to confirm all of your medications and doses.

EMERGENCY

In an emergency situation you should go directly to the hospital emergency room where a physician is immediately available at all times. Treatment will be initiated and then we will be contacted. For non-emergency concerns during office hours, call our office telephone number (301) 695-6800. The receptionist or voice mail system will contact your doctor with your message. Please use the physician on-call option for urgent messages only and remember to leave your phone number; otherwise, the doctor cannot return your call. If you do not receive a call back within 30 minutes, please call the Holy Cross Germantown Hospital and request to have the Frederick Gastroenterology on-call physician paged.

Policy on Second Opinion

FGA encourages second opinions from qualified Gastroenterology experts when needed. If Frederick Gastroenterology Associates refers you for a second opinion you have the choice of continuing your care here or following with the Gastroenterology expert. If you chose another Gastroenterology opinion locally without our referral we ask that you transfer your care to that Gastroenterology office.
